Anthony D. Barak, Esquire

Anthony (Tony) Barak is one of an elite group of attorneys in the State of Florida who achieved Florida Bar Board Certification in Health Law in 2008 and was re-certified in 2013 and in 2018. Certification is the highest level of evaluation by the Florida Bar of Competency and experience in the specific area of law approved for certification by the Supreme Court of Florida. Tony’s practice areas encompass many aspects of health law and insurance disputes, including personal injury protection (PIP) claims and accident claims including, but not limited to, automobile/motorcycle accidents, wrongful death claims, and other general civil litigation matters.


Tony has won and has published, 12 decisions on behalf of medical providers against insurance companies, including a decision from the Florida Second District Court of Appeals. Tony has lectured to medical providers and to attorneys regarding Florida’s No-Fault Law (PIP statute). Tony is an experienced litigator and has represented medical providers and insureds in the pursuit of justice against insurance companies in thousands of cases throughout his career.


Prior to founding Barak Law Group, P.A., Tony was the founding partner at Barak & Goldberg, P.A., which involved the same areas of practice. Additionally, Tony has previously served as corporate counsel for a large group medical practice with over 40 offices throughout the State of Florida and over 400 employees. The company was accredited by the Joint Commission on Accreditation of Healthcare Organizations. Tony assisted the company in obtaining its Joint Commission Accreditation, advised the company on various health law-related matters, and handled litigation matters against insurance companies on first-party insurance disputes. 


Tony received his Juris Doctor degree from Nova Southeastern University Law School in 2000 where he served on Law Review. Prior to attending law school, Tony received his Bachelor of Science degree from the University of Florida where he majored in psychology with a minor in criminal law in 1996. He is a member of the health law section and trial lawyers section of the Florida Bar, a member of the Sarasota County Bar Association, Manatee County Bar Association, and a member of the Manatee/Sarasota Trial Lawyers Association. Tony is admitted to practice before all State Courts of Florida and the United States Middle District Court of Florida.

Brian Forman is a Florida attorney that started practicing in 1995 and is also a licensed Florida adjuster. His current practice captures multiple areas of health and first-party insurance law. Some of Brian’s concentrations have been in SIU and claims investigations for many years. While working in the SIU field, the National Society of Professional Insurance Investigators awarded Brian the “F. Lee Brininger Award” for Investigator of the Year. He was also nominated for the investigator of the year at the Florida Insurance Fraud Education Committee on two different occasions, as well as acting as defense counsel for standard and non-standard insurers throughout Florida. As his career progressed, he gradually shifting to almost exclusively representing medical providers in first-party personal injury protection cases culminating in representing a large group practice that had over 50 Joint Commission-accredited offices across Florida.

 

In 1995, Brian earned his Juris Doctorate from Saint Thomas University School of Law in Miami, Florida where he was on Trial Court and worked in the Landlord-Tenant clinic representing tenants. Prior to law school, Brian earned his Bachelor of Arts in both Russian Language and International Relations with an emphasis on the Soviet Union and Eastern Europe from Florida State University. Brian also earned a Certification in Health Care Risk Management from the University of Central Florida. 

Attorney M. Brian Crim

Since 2003 attorney M. Brian Crim has concentrated his practice in the areas of Personal Injury Protection insurance and first-party homeowner's insurance disputes, representing claimants exclusively in such matters since 2009.


Mr. Crim is admitted to practice before all Florida state courts, as well as the United States District Court for the Middle District of Florida. He is an experienced litigator who has represented a wide variety of clients in hundreds of court cases and appears in the county, circuit, Federal, and appellate courts of the Tampa Bay area and indeed, around the state. In addition, he is equipped to assist clients from the inception of their claims to the appeals courts if need be.


He began his career defending insurance companies, giving him a thorough understanding of how insurance companies think and approach defending cases. Mr. Crim now primarily represents consumers, policyholders, and other claimants in disputes with insurance companies over wrongly denied or reduced claims. Insurance policies and the applicable statutes that govern them are a complicated labyrinth and Mr. Crim's many years of experience provide valuable insight into how to navigate them.


Prior to college Mr. Crim attended Bloomingdale High School in Valrico and earned the rank of Eagle Scout in the Boy Scouts of America. Mr. Crim received a Bachelor of Science degree with a double major in Political Science and Psychology from Florida State University before attending the College of Law at the University of North Carolina at Chapel Hill where he obtained his Juris Doctor in 2002.
